I will examine two machines (two doors) and the two tragedies that resulted from doors that would not open when they needed to be opened. The resulting failures resulted in the deaths of three astronauts in Apollo 1 on January 27, 1967 and 150 passengers and crew onboard Germanwings Flight 9525 on March 24, 2015. Human Factors Engineering (HFE) should have played a more significant role in risk reduction for both events. These events would have had better outcomes, i.e., no loss of human life, if areas of concern had been implemented as preliminary requirements prior to system validation. Read more →
